Objectives – Participants will be able to:

1.Understand risk factors for and identification of patients with CKD

2.Review management of co-morbidities of CKD

3.Understand the role of multidisciplinary CKD clinics in co-management of CKD

Anton C. Schoolwerth, MD, MSHA, is Professor of Medicine and physician at Dartmouth-Hitchcock Medical Center in Lebanon, NH. He received his MD from Harvard Medical School and Nephrology training at Washington University School of Medicine in St. Louis, MO. He has served as the founding Chief, Renal and Electrolyte Division, Pennsylvania State University School of Medicine in Hershey, PA, and as Chairman, Division of Nephrology and Vice-Chairman, Department of Medicine, at Virginia Commonwealth University School of Medicine in Richmond, VA.  At VCU, he received a Master of Science in Health Administration degree.

Dr. Schoolwerth has served in various positions with the National Kidney Foundation, including as a member of the Advisory Board on Outcomes Quality Initiative (K/DOQI), a member of the National Medical Advisory Board, as Chairman of the K/DOQI Kidney Learning System, as Editor-in-Chief, Best Practices Newsletter, and as President, National Kidney Foundation of the Virginias. He has served as a member and Chairman of the Executive Committee of the Council on the Kidney in Cardiovascular Disease for the American Heart Association, and on the ASN Training Program Directors’ Executive Committee for the American Society of Nephrology (ASN). He has been a member of General Medicine B Study Section, as well as the Professional Workgroup, National Kidney Disease Education Program for the National Institutes of Health. He has served as a renal member of the National Merit Review Board for the Veteran’s Administration. In addition, he has served at the Centers for Disease Control and Prevention as member of the Kidney Disease Workgroup on Public Health Strategies on Chronic Kidney Disease (CKD) and as a member of the Committee on the Economic Impact of CKD in the U.S., both in the Division of Diabetes Translation.  Dr. Schoolwerth has served on numerous editorial boards and has published over 140 articles and book chapters in medical literature.
